COVID-19 EMERGENCY RENTAL ASSISTANCE
HASNOK will be taking applications from October 31st – December 20th, 2022. This will be a one-time, first come-first serve Rental Assistance. Assistance will be provided up to 3 months only (if applicable).
Please Print and Submit application to the Housing Authority office located at 120 W. 4th St. Wewoka, OK. 74884
Applications will not be accepted unless all documents are attached. Please make sure you have everything that is listed when turning in application
Payments will only be made directly to the landlord.
o 1 or more individuals within the household has qualified for unemployment benefits or experienced a reduction in household income, incurred significant costs, or experienced other financial hardship due, directly, or indirectly, to the COVID–19 outbreak.
o 1 or more individuals within the household can demonstrate a risk of experiencing homelessness or housing instability.
o Household income is at or below 80% of area median income.
o Completed Application
o Copy of Tribal Membership Card for Head of Household or Spouse (if applicable)
o Copy of State Driver’s License or ID
o Copy of Social Security Card (all household members)
o Landlord/Rental Form
o Tax Identification Number required
o Copy of Lease
o Income Verification (please submit one of the following for all household members receiving income):
o 30 Days Check Stubs
o 2022-2023 Award Letters
o Proof of Unemployment (Letter from Workforce office)
